Qualifications

1.        All participants should be aged 18 or above and must sign a disclaimer form before participation.

2.           In team format with the following category awards.
Corporate Trophy (In team of 4) - Teams should be enrolled under a company’s name with the minimum donation of HK$20,000 per team.  If 3 teams or above are enrolled, the minimum donation per team will be adjusted to HK$15,000, and the company’s name will be acknowledged in the souvenir programme.
Open Trophy (In team of 4) - Teams should be formed by 4 individuals, with the minimum donation of HK$6,000 per team.
Student Trophy (In team of 4) - Teams should be formed by 4 students aged 18 or above, with the minimum donation of HK$4,000 per team.
Pair Trophy (In team of 2) - Team should be formed by 2 individuals, with the minimum donation of HK$4,000 per team.

 Enrollment

1.        The enrollment deadline is 6 October 2009 (Tuesday).  Application will be served in first-come-first-serve basis. Each participant is required to donate HK$100 as registration fee for qualification.  The registration fee will be included in the minimum donation.  All donations are non-refundable.

2.         Application method:
- Online application
- By email, fax, mail or in person: Please complete the enrollment form with the registration fee and return to The Salvation Army Headquarter.

3.        Upon confirmation of registration, confirmation letter, sponsorship form and information of the briefing session will be email to the team leaders (Please provide effective email address).  All the information provided should be distributed to the members by the team leaders.  Photocopies of the forms are welcomed.

4.        Please make cheque crossed payable to "The Salvation Army" (Please mark "HK & Macau O! Day" at the back), or deposit to the Army's HSBC account (143-2-013116), and return the original copy of bank deposit slip or the print-out of online transaction confirmation (Please put the names of the participants on the back for reference), along with the enrolment form to The Salvation Army.

5.        All participating teams should submit all donations (included the minimum donation or more) on or before 6 October 2009 (Tuesday) for qualification.  Teams can strive for the Personal Top Fundraising Team Award by raising extra donation. Official receipts will be issued to the team leaders for distribution to all members.
