Qualifications

  1. All participants should be aged 18 or above and must sign a disclaimer form before participating.
     

  2. In team format (4 members in a team) and with the following categories and awards:
    Corporate Team - Teams should be enrolled under a company’s name with minimum donation of $20,000 per team. For companies that enrol 3 teams or above, the minimum donation for each team is $15,000. The company’s name will be acknowledged in the souvenir programme.
    Open Team - Teams should be formed by individuals, the minimum donation is $6,000 per team.
    Student Team -
     For students with minimum donation of  $4,000 per team.

Enrollment

  1. Please complete and return the enrollment form to The Salvation Army by email, fax, mail or in person.  Upon confirmation of registration, team leaders will receive sponsorship forms which should be distributed to their members. Photocopies of the forms are welcome. On-line registration is also accepted. (Enrollment was ended. Thank you for your participation.)
     

  2. Each participant is requested to donate $100 as registration fee.  Donations should be submitted to The Salvation Army on or before 14 October 2008 to be qualified for the event.
     

  3. All donations are non-refundable.  Please make cheque crossed payable to "The Salvation Army" and mark "HK & Macau O! Day" at the back of the cheque; or deposit the donation to the Army's HSBC account 143-2-013116, and return the original copy of bank deposit slip or print the confirmation for online transaction, along with the enrollment form.
     

  4. Official receipts will be issued to the team leaders for distribution.
     

  5. Enrollment deadline is 6 October 2008 (Monday).
